Telehealth Maternity Care Program

Jackson Health System’s Telehealth Maternity Care Program, RHISE, provides support throughout pregnancy and up to 12 months after your baby is born — right from your home. Our team is here to help you stay healthy, supported, and connected.

Who Can Join

Pregnant or postpartum women (up to 12 months after giving birth)

  • This program supports patients with high-risk pregnancy conditions, such as high-blood pressure, diabetes, or identified social needs
  • Participants must live in Miami-Dade County or one of our partner counties

How It Works

  1. We receive a referral from your doctor or community partner
  2. You are enrolled in the program and contacted with next steps
  3. If needed, we provide the equipment required for at home monitoring
  4. You receive virtual support and submit daily health readings
  5. You continue to receive ongoing support throughout the first year postpartum
